Applying for a Service

All applications for service provision are considered together with the HSE.

A referral for Saint John of God Liffey Services must be made in writing to:

A referral can be made by:

Yourself

A parent

the HSE

A GP

Any Multidisciplinary Team clinician, for example a Psychologist, Occupational Therapist, Speech and Language Therapist

A Consultant Paediatrician

Referrals must include the most recent assessment report, for example Occupational Therapy, Speech and Language Therapy, Psychology, Paediatrics.

When we get the referral, we will send back an acknowledgment letter to the person who made the referral.

The referral is then placed on the agenda of the Application for Support Committee, to be discussed.

The Application for Support Committee is supported by the

Early Services Committee

Residential Committee

Respite Committee

After the meeting, we inform the person who made the referral what  the outcome of the application is.

To get respite in Saint John of God Liffey Services, the person must already be getting a service from Saint John of God Liffey Services.

Any request for respite must have a Respite Application form.

The Respite Manager will help the referrer to complete the All About Me Assessment form.

Respite Applications are also discussed as part of the Application for Support Committee.
